Join us in person for this year’s Connected Care Summit: Connected Care, Collective Power
Building on the strong participation and energy from last year’s inaugural event, the 2nd Annual 504HealthNet Connected Care Summit continues this momentum by strengthening collaboration and advancing coordinated, patient-centered care across Greater New Orleans. With ongoing Medicaid eligibility changes, persistent barriers to primary and healthcare, and high rates of chronic disease across the region, this year’s Summit focuses on addressing the real challenges residents face in accessing timely, high-quality care.
The Summit elevates local innovation, highlights the work of 504HealthNet member organizations, and creates shared strategies that improve connections between medical care, behavioral health, and community-based services. Through facilitated discussions, interactive activities, and member showcases, participants will leave with actionable approaches to improving coordinated care and advancing access across Southeast Louisiana.
