This in-person event brought together 100 community-based healthcare professionals, community partners, and local leaders to foster collaboration across organizations to break down silos and increase access to care. The current climate demands that we come together, share our experiences, and build a future where everyone has access to the care they need, together. The Summit proved how vitally necessary it is that we convene, build community, learn from the past and strategize into the future.
The Connected Care Summit will be an annual convening that embodies 504HealthNet’s mission and sparks action to fully realize a connected and coordinated network of community health care providers.
